You will find the following forms useful:

• The Admissions form includes a Statement of Policy and Agreement to be signed by all applying families.
• The Application Checklist outlines the steps families take in applying to St. Ambrose, including students transferring from other high schools.
• The Re-enrollment Form is for existing St. Ambrose families to apply for the next year.
• The Student Application Form is to be filled out for each applying student.
• Tuition and Fees provides the tuition and fee schedule for tuition, books, electives fees, and the tuition reduction schedule for siblings.
• The Tuition Contract allows families to choose a payment plan that works for them.

Scholarships are awarded on an annual basis. St. Ambrose uses an independent needs-assessment service to determine a family's scholarship needs. The financial aid procedures begin after a student is admitted to St. Ambrose Academy. Our goal is that no qualified student be turned away from St. Ambrose for want of funds.
